Plan Terms

Membership details

  • Each member must leave a valid credit card on file for auto-renewal.
  • If the charge is denied, you have 15 days to make alternative payment before lapse.
  • Lapsed memberships incur a $25 reactivation fee. Lapsed more than twice — future sign-up is prohibited.
  • Unused benefits expire once membership expires.
  • Only uninsured patients are eligible to apply.
  • Membership is not transferable.
  • No refund once any service item in the membership benefits has been used.

Common questions

No. Centre Dental discount plans are only available to uninsured patients. If you have any active dental insurance, you cannot enroll. If your coverage ends, you become eligible.
Up to 20% off procedures completed at Centre Dental beyond what's already included in the plan — e.g., fillings, crowns, implants, veneers, Invisalign. The exact discount varies by procedure.

Benefits reset annually on your enrollment anniversary. Unused visits do not carry over — we'll remind you as your renewal date approaches.
